About the Advanced Cardiac Sonographer (ACS) exam

The CCI Advanced Cardiac Sonographer examination assesses the knowledge and skills used by sonographers working at an advanced level in the echocardiography laboratory. Use this short diagnostic to identify topics that deserve more review, then compare your results with the official examination matrix.

ACS examination matrix

The percentages below are the approximate exam-score weights published by Cardiovascular Credentialing International (CCI). Use them to allocate study time, then adjust for your own weak areas.

10%

Assimilating History, Clinical, and Echocardiographic Findings

Integrate the clinical picture with images and preliminary reporting, communicate significant findings, and resolve discrepancies.

  • Integrate patient assessment, clinical history, and comparative studies
  • Review preliminary reports and images for completeness and accuracy
  • Communicate significant findings and reconcile report-image discrepancies

39%

Assessing Cardiovascular Heart Disease

The largest domain spans acquired heart disease, hemodynamics, stress echo, ventricular function, support devices, and special populations.

  • Assess cardiomyopathies, coronary disease, pericardial disease, heart failure, aortic disease, and cardiac masses
  • Assess the right heart, pulmonary hypertension, systemic disease, and special populations
  • Evaluate mechanical circulatory support, measurements, hemodynamics, and diastolic function

18%

Assessing Valvular Heart Disease

Characterize native and prosthetic valve disease and recognize findings associated with valvular endocarditis.

  • Evaluate aortic, mitral, pulmonic, and tricuspid valve disease
  • Evaluate prosthetic valves and valvular endocarditis

17%

Evaluating Structural Heart and Interventional Procedures

Support patient screening, peri-procedural guidance, follow-up imaging, multimodality comparison, and complication recognition.

  • Screen for structural intervention and perform peri-procedural imaging analysis
  • Perform follow-up analysis and compare multimodality imaging
  • Evaluate interventional and surgical complications

11%

Utilizing Advanced and Multi-Modality Imaging

Acquire and analyze advanced echo datasets and integrate findings from other cardiac imaging modalities.

  • Acquire and analyze 3D and multiplanar reconstruction datasets
  • Acquire and analyze LV and RV strain imaging
  • Integrate CMR, cardiac CT, fluoroscopy, nuclear/PET, and ICE imaging

5%

Assessing Congenital Heart Disease

Assess simple and complex congenital heart disease before and after repair.

  • Assess lesions such as ASD, VSD, PDA, PFO, persistent left SVC, and coarctation
  • Assess complex disease such as TGA, tetralogy of Fallot, tricuspid atresia, and HLHS

How to use this ACS diagnostic

The 15 questions sample all six official domains in roughly similar proportions to the matrix. Because the set is short, your percentage can change substantially with one answer. Treat the result as a starting point: review every explanation, record the reasoning gap behind uncertain answers, and follow with larger mixed sets.

High-yield ACS study priorities

Priority 1

Start with cardiovascular heart disease

At 39% of the examination matrix, this domain deserves the largest share of an initial study plan. Practise synthesizing measurements and clinical context rather than memorizing isolated thresholds.

Priority 2

Connect valve assessment with hemodynamics

Review how loading conditions, Doppler alignment, flow state, and measurement error can alter apparent severity. Practise reconciling discordant findings before assigning a conclusion.

Priority 3

Rehearse procedural imaging decisions

Structural and interventional content tests what to acquire, how to orient views, which measurements matter, and when a complication needs immediate communication.

Priority 4

Use multimodality cases

Build cases that require you to compare echo with CT, CMR, fluoroscopy, nuclear/PET, or ICE rather than studying each modality as a separate list.

ACS exam FAQ

How many questions are on the CCI ACS exam?

CCI currently lists 170 questions: 150 scored questions and 20 unscored questions that are not identified during the examination.

How long is the ACS exam?

The exam appointment is three hours. CCI allocates 2 hours 50 minutes to answer questions and 10 minutes to the tutorial and post-exam survey.

What is the largest ACS exam domain?

Assessing Cardiovascular Heart Disease is the largest domain at approximately 39% of the exam score.

Does this diagnostic predict whether I will pass?

No. A 15-question diagnostic is a low-confidence baseline, not a validated pass prediction. Use it to find review priorities and confirm current exam requirements with CCI.

Is MedPorium affiliated with CCI?

No. MedPorium is an independent education platform and is not affiliated with or endorsed by Cardiovascular Credentialing International.
