About the Certified Medical-Surgical Registered Nurse (CMSRN) exam

The CMSRN examination assesses medical-surgical nursing practice across safe patient management, holistic care, interprofessional coordination, professional concepts, and nursing teamwork and leadership. Strong preparation combines bedside clinical judgment with communication, transitions, ethics, quality, delegation, and systems thinking rather than treating them as separate topics.

MedPorium is an independent education platform and is not affiliated with or endorsed by Medical-Surgical Nursing Certification Board (MSNCB). Exam details can change; confirm them with the official source before applying.

CMSRN examination matrix

The percentages below are the approximate exam-score weights published by Medical-Surgical Nursing Certification Board (MSNCB). Use them to allocate study time, then adjust for your own weak areas.

32%

Patient/Care Management

The largest domain covers safe bedside management through nursing process, risk reduction, infection prevention, medications, pain, procedures, and nutrition.

  • Apply assessment, diagnosis, planning, implementation, and evaluation while prioritizing immediate and emerging safety risks
  • Use infection-control standards, medication safeguards, vascular-access care, pain strategies, and appropriate nonpharmacologic interventions
  • Prepare for procedures, recognize complications, practise within sedation and procedural scope, and individualize nutritional support

15%

Holistic Patient Care

Provide patient-centered, culturally and linguistically responsive education, advocacy, health promotion, and palliative or end-of-life care.

  • Elicit goals and preferences, involve families appropriately, advocate, and respond constructively to concerns or grievances
  • Address culture, language, disability, bias, health literacy, learning needs, prevention, and health-promotion goals
  • Support palliative resources, advance directives, code status, post-mortem care, organ donation, and required death reporting

17%

Elements of Interprofessional Care

Coordinate individualized care and transitions through effective teamwork, documentation, community resources, outcome measurement, and technology.

  • Use clinical judgment and collaborative problem solving during interprofessional rounds and care planning
  • Coordinate discharge, reconciliation, mobility, community support, family-centered care, readmission prevention, and continuum transitions
  • Document accurately, follow downtime procedures, coach improvement, and use or troubleshoot health technology and informatics

15%

Professional Concepts

Apply communication, critical thinking, ethical and legal scope, healthy-workplace, quality, evidence, and research principles.

  • Use chain of command, structured handoffs, closed-loop communication, interpreters, conflict resolution, and de-escalation
  • Prioritize care, recognize deterioration, mobilize crisis resources, and use disciplined analysis and problem solving
  • Apply ethics, patient rights, regulation, professional reporting, quality improvement, adverse-event learning, evidence, and research concepts

21%

Nursing Teamwork and Collaboration

Delegate and supervise safely while supporting competency, professional growth, leadership, organizational performance, and disaster readiness.

  • Match delegated care to patient condition, task complexity, team-member scope, competency, supervision needs, and available resources
  • Use mentoring, coaching, reflective practice, precepting, peer review, education, and professional engagement to build team capability
  • Apply shared governance, leadership and change models, staffing and fiscal stewardship, conflict management, emergency procedures, and incident command

How to use this CMSRN diagnostic

The 15-question diagnostic samples all five official blueprint domains. Because the set is short, use the result as a low-confidence baseline: classify every miss by domain, identify whether the gap involved assessment, prioritization, intervention, communication, coordination, safety, or scope, then confirm the pattern with larger mixed and domain-specific sets.

High-yield CMSRN study priorities

Priority 1

Lead with patient and care management

At 32% of scored content, this domain should receive the largest share of practice. Use unfolding cases that require assessment, prioritization, safe intervention, reassessment, and escalation rather than isolated fact recall.

Priority 2

Practise delegation as a complete decision

For every assignment, consider stability, predictability, task complexity, licensed or unlicensed scope, demonstrated competency, supervision, follow-up, and what the RN must retain.

Priority 3

Connect transitions with social context

Discharge questions often hinge on reconciliation, readiness, health literacy, caregivers, mobility, equipment, community resources, affordability, social determinants, and who owns the next step.

Priority 4

Rehearse escalation and systems thinking

Practise recognizing deterioration, using the chain of command, communicating with SBAR and closed loop techniques, reporting near misses, and choosing system-level quality responses without exceeding nursing scope.
