About the Oncology Certified Nurse (OCN) exam

OCN assesses the knowledge used in adult oncology nursing across prevention, diagnosis, treatment, symptom management, emergencies, psychosocial care, survivorship, palliative care, and professional practice. Strong preparation links cancer biology and treatment mechanisms with nursing assessment, safe administration, monitoring, education, coordination, and timely escalation.

OCN examination matrix

The percentages below are the approximate exam-score weights published by Oncology Nursing Certification Corporation (ONCC). Use them to allocate study time, then adjust for your own weak areas.

14%

Care Continuum

Support prevention, screening, navigation, survivorship, palliation, advance planning, and end-of-life care across the cancer trajectory.

  • Apply epidemiology, risk reduction, screening, and early-detection principles
  • Coordinate navigation, rehabilitation, surveillance, recurrence concerns, and delayed effects
  • Support goals of care, advance directives, hospice, bereavement, caregivers, and comfort

15%

Oncology Nursing Practice

Apply cancer science, diagnostic and prognostic information, professional standards, safety, ethics, advocacy, and culturally congruent care.

  • Relate carcinogenesis, immunology, genetics, genomics, diagnostics, staging, grading, metastasis, and performance status
  • Apply research, clinical-trial, documentation, accreditation, scope, and standards principles
  • Use safe handling, PPE, ethical reasoning, advocacy, collaboration, culture, and professional self-care

20%

Treatment Modalities

Provide safe nursing care and education across surgical, radiation, systemic, localized, targeted, cellular, transplant, and oral therapies.

  • Prepare, educate, assess, and monitor patients receiving procedures, surgery, radiation, and transplant
  • Apply nursing principles for chemotherapy, endocrine, immunotherapy, targeted, bispecific, cellular, intrathecal, intravesical, and oral therapy
  • Recognize expected response, common toxicities, administration risks, and findings requiring escalation

25%

Symptom Management and Supportive Care

The largest domain covers treatment effects, system alterations, vascular access, pain, nutrition, rehabilitation, and supportive interventions.

  • Assess acute, chronic, and delayed symptoms across body systems
  • Manage vascular access, hematologic and immune effects, pain, nutrition, function, and treatment-related alterations within nursing scope
  • Use pharmacologic, nonpharmacologic, complementary, palliative, educational, and interdisciplinary support

16%

Oncologic Emergencies

Recognize high-risk oncologic and treatment emergencies, initiate immediate nursing actions, and escalate promptly.

  • Recognize sepsis, DIC, tumor lysis, SIADH, hypercalcemia, tamponade, obstruction, VTE, cord compression, SVC syndrome, and increased ICP
  • Respond to anaphylaxis, hypersensitivity, extravasation, pneumonitis, immune toxicities, CRS, and ICANS
  • Prioritize stabilization, treatment interruption when indicated, focused assessment, monitoring, communication, and emergency activation

10%

Psychosocial Dimensions of Care

Address culture, spirituality, distress, coping, family dynamics, sexuality, fertility, crisis, learning, autonomy, caregiving, and financial burden.

  • Assess distress, depression, grief, crisis, suicide risk, violence, caregiver fatigue, and support needs
  • Provide culturally responsive education and adapt for literacy, language, preference, and access barriers
  • Support body image, intimacy, fertility, sexuality, gender diversity, autonomy, resources, and family coping

How to use this OCN diagnostic

The 15 questions sample all six official domains, with the largest share devoted to Symptom Management and Supportive Care. Classify each gap as continuum, biology and diagnosis, treatment, symptom or supportive care, emergency recognition, psychosocial care, safety, education, or professional practice instead of relying on the overall percentage alone.

High-yield OCN study priorities

Priority 1

Give symptom care the largest share

Use system-based cases that require cause, assessment, prevention, intervention, teaching, reassessment, and escalation rather than memorizing toxicity lists.

Priority 2

Pair every therapy with nursing surveillance

For each modality, know preparation, administration safety, expected effects, early toxicity, delayed toxicity, patient education, and the finding that changes the plan.

Priority 3

Rehearse emergencies as first-action cases

Practise recognizing the pattern, protecting airway and circulation, stopping a causative infusion when appropriate, obtaining focused data, escalating, and monitoring response.

Priority 4

Follow patients beyond active treatment

Include recurrence surveillance, late effects, rehabilitation, sexuality, psychosocial recovery, caregiver needs, palliation, and goals of care in mixed practice.

OCN exam FAQ

How many questions are on the OCN exam?

ONCC currently lists 165 multiple-choice questions. Of these, 125 count toward the score and 40 are unscored pretest questions that cannot be identified during the examination.

How long is the OCN exam?

The test session is three hours, including 15 minutes for the computer-based testing tutorial before the exam and a post-test survey. There are no scheduled breaks, and the timer continues during an unscheduled break.

How is the OCN content outline weighted?

The 2026 outline is weighted Care Continuum 14%, Oncology Nursing Practice 15%, Treatment Modalities 20%, Symptom Management and Supportive Care 25%, Oncologic Emergencies 16%, Psychosocial Dimensions of Care 10%. The percentages apply to the 125 scored questions.

How long is the OCN testing window?

ONCC currently provides a 90-day testing window after authorization to test. Candidates should confirm processing times, authorization dates, and scheduling instructions directly with ONCC.

Who should verify eligibility for OCN?

Candidates should confirm the current license, education, experience, practice-hour, and continuing-education requirements on the official ONCC page before applying. This study page does not determine eligibility.

Does this diagnostic predict whether I will pass?

No. A 15-question diagnostic is a low-confidence baseline, not a validated pass prediction. Review every rationale, then use larger mixed and domain-specific sets to build a more stable readiness estimate.

Is MedPorium affiliated with ONCC?

No. MedPorium is an independent education platform and is not affiliated with or endorsed by the Oncology Nursing Certification Corporation.
